LATE REGISTRATION REQUIREMENTS

BIRTH

• Affidavit of 2 disinterested person. (New CTC)
• Negative (NSO).
• At least 2 valid documents showing the place of birth.(baptismal, voter’s registration record etc..)

MARRIAGE

• Affidavit of 2 disinterested person. (New CTC)
• Negative (NSO).
• Copy of the unregistered document.

DEATH

• Affidavit of 2 disinterested person. (New CTC)
• Negative (NSO).
• Copy of the unregistered document.
